Premium Medical Services

This ONE-TIME fee is charged for each new patient at the time of scheduling an appointment.

Includes:

• Set up of chart
• Review of previous records, if applicable
• Coordination of care with other providers, when necessary
• Direct contact with the nurse practitioner surrounding appointments via email

*This non-refundable fee is in addition to the In-Home Visit Fee

In-Home Visit Types:

• Medical Evaluation
• Medical Advice 
• Newborn/Well Child Check-ups

A virtual medical evaluation on our secure, HIPAA compliant, tele-med platform. 

Available as an option for established patients only.

• Discussion of medical questions
• Parenting support
• Diagnosis education and recommendations
• Treatment decision guidance

*All of these services are free when in conjuncture with a Summit appointment

• Filling out of medical letters and sports physicals
• Calling in medical prescriptions

This appointment Is for IV hydration in the comfort of your home with one of our licensed registered nurses. 

Fluids and electrolytes quickly rehydrate and promote healing. 

IV fluids are beneficial for dehydration from vomiting, jet lag, and overexertion.

Referral required. Appointments are limited.

Adult patients only.

Our primary coverage area at this time is Frederick, Maryland

(15 mile radius of 21704 zip code).

When we are available, we will travel outside of that range and this fee covers that expense.

16 - 29 miles from 21704 = $50

30 - 45 miles from 21704 = $75

46 + miles from 21704 = $100
